At Forman, we believe that insight leads to empowerment. The Quinn Assessment Center reflects this philosophy, offering evaluations that don’t just diagnose but also guide next steps—helping students and adults thrive academically, socially, and personally.
We offer three levels of evaluation services, tailored to the needs of each individual:
Neuropsychological Evaluations: In-depth assessment of cognitive, academic, social-emotional, and executive functioning skills to provide a holistic profile and robust recommendations.
Diagnostic Evaluations: Focused assessment to clarify or confirm specific diagnoses (e.g., ADHD, ASD, anxiety, learning disorders).
Psychoeducational Evaluations: Targeted assessments to guide educational planning, including academic accommodations and individualized learning supports.
Across all evaluations, we emphasize not only identifying challenges but also highlighting strengths and strategies to promote growth.
Families and individuals leave with:
A clear, user-friendly written report explaining results in accessible terms.
Practical recommendations for home, school, and community.
A collaborative feedback session to ensure understanding and action planning
Appointments are available to both Forman families and the general public. We recognize that accessibility and collaboration are key to a successful evaluation process. To meet families where they are, our clinicians can provide services on the Forman campus, travel to homes, and conduct assessments in school settings. We also work closely with local districts to ensure findings are integrated into educational planning, making the process as seamless and supportive as possible for students and families.
We want the assessment process to feel supportive, transparent, and convenient. Here’s what families and individuals can expect:
Flexible Scheduling: Testing sessions are offered at times that work best for families and students—including evenings or weekends when needed—to minimize disruption to school or work.
Testing Environment Options: Assessments can be conducted at the Quinn Assessment Center, in schools, or in-home, depending on the learner’s needs and comfort.
Assessment Days: Most evaluations are completed over one to two days, with scheduled breaks to support focus and comfort. Younger children often benefit from shorter sessions spread out over time.
Quick Turnaround: We understand that families and schools need answers promptly. Feedback sessions are typically scheduled within two weeks of testing, with a comprehensive written report provided shortly thereafter.
Collaborative Process: We encourage input from parents, educators, and outside providers so results are both accurate and actionable.
This approach allows us to adapt to the unique needs of each individual while ensuring the evaluation process remains thorough, efficient, and family-centered.

Evaluation fees are as follows:
Diagnostic Assessment: $2,500
Psychoeducational Assessment: $3,000
Neuropsychological Assessment: $6,000
We are not accepting insurance at this time; however, clients may request reimbursement from their insurance provider for out-of-network services.”
